Geriatrics and Extended Care

About the Program

The Geriatrics and Extended Care Program at VA NJ Healthcare System is composed of five areas —

  • Lyons Nursing Home Care Unit
  • Home Based Primary Care
  • Community Care Programs
  • Palliative Care Program
  • State Veterans Home Program

Services Offered

Lyons Nursing Home Care Unit (NHCU)

NHCU services include —

  • occupational therapy
  • physical therapy
  • speech therapy
  • recreation therapy
  • specialized services to veterans in need of rehabilitation following amputation, stroke, traumatic brain injury, and spinal cord injury

Home Based Primary Care

This program provides primary care to veterans who are homebound or for whom travel to VA NJ's primary care outpatient clinics is a hardship.

Community Care Programs (CCP)

CCP consists of programs that provide care in veterans' homes and community settings.  Services include:

  • adult day health care
  • community nursing home care
  • skilled and unsilled home care
  • home hospice care
  • home IV therapy

Palliative Care Program

Palliative Care provides comfort care for those at the end-of-life.  Palliative Care Consultation teams are available at East Orange and Lyons for symptom management at any stage of a patient's illness.

State Veterans Home Program

Under this program, VA NJ inspects the two New Jersey state nursing homes for veterans in Menlo Park and Paramus, and provides funding to support veterans stays at those homes.
